Disseminated CNS lesions

If disseminated CNS lesions are seen on CT then the differential includes:-

  • multiple infarcts, multiple sclerosis, meningovascular syphilis, multiple metastases, subacute combined degeneration of the cord, Freidreich's ataxia, tabes dorsalis, neuromyelitis optica, diffuse sclerosis, progressive multifocal leukoencephalopathy

This list may be classified using a surgical sieve as:-

  • congenital causes - Friedreich's ataxia
  • acquired causes (INVITEDMD) - infection - progressive multifocal leukoencephalopathy, syphilis (tabes dorsalis, meningovascular syphilis) - neoplasm - multiple metastases - vascular - multiple infarcts - inflammatory - neuromyelitis optica - trauma - none - endocrine - metabolic - subacute combined degeneration of the spinal cord - degenerative - multiple sclerosis, diffuse sclerosis
