The testes may be damaged as a result of a direct blow to the scrotum, for example during contact sports and fights.
Patients present with an acutely painful testis. If the testis has not ruptured, a large haematoma in the scrotum may be felt on examination; if the testis has ruptured, the tunica vaginalis will be full of blood forming a haematocoele.
Early surgical exploration is indicated if there is severe and persistent pain. Incision of the tunica vaginalis may relieve the pain associated with a haematoma. A testis which is badly damaged must be removed.
