Cautions:

  • diabetes
  • gout
  • may exacerbate systemic lupus erythematosus
  • pregnancy and breast feeding
  • ineffectual if renal impairment - serum creatinine > 150 micromol/litre; use a loop diuretic instead
  • hepatic impairment
  • hyperlipidaemia is not a contraindication because the long-term effect of low doses on lipids is trivial

Contra-indications

  • refractory hyponatraemia, hypokalaemia, hypercalcaemia
  • severe hepatic and renal impairment
  • symptomatic hyperuricaemia
  • Addison's disease
