If the appropriate criteria are satisfied then give streptokinase as follows:
1.5 million units in 100 ml 0.9% saline i.v. over one hour and aspirin 150 mg orally. Also some start a heparin infusion (1000 units/hour) four to six hours after the streptokinase. This is continued for 24 hours.
The aspirin, 150 mg orally daily, should be continued indefinitely.
Evidence from the GUSTO study suggests that accelerated tPA with heparin is a more effective regime than the one described above.
