The most common cause is lymph node metastases from primary bronchial carcinoma.

Other causes include:

  • lymphadenopathy due to lymphoma
  • thymoma
  • mediastinal tumour
  • retrosternal goitre
  • mediastinal fibrosis
  • thoracic aortic aneurysm

In children the most important causes are:

  • T cell leukaemia
  • lymphoma
