Individuals at risk either live close to or have visited endemic areas. The species of Babesia vary according to geographical location.

In Northeastern United States Babesia microti is transmitted by the Ixodes tick.

In California and Washington the organism is similar to Babesia gibsoni. The vector is not known.

In Europe, Babesia divergens is transmitted by the Ixodes tick.

Cases have also been reported following transfusion of blood from donors that either live in or have visited endemic areas.
