Endemic syphilis is caused by the endemicum subspecies of Treponema palliudum and, like yaws and pinta, results from direct inoculation through broken skin.

This condition particulary affects children in Saharan Africa and the Middle East, and can be very difficult to distinguish from classical - veneral - syphilis.

Treatment is with penicillin intramuscularly.
