Maydl's hernia describes when two adjacent loops of small intestine are within a hernial sac with a tight neck. The intervening portion of bowel within the abdomen is deprived of its blood supply and eventually becomes necrotic.

Symptoms and signs can include:

  • intestinal obstruction
  • hernia:
    • tense
    • slightly tender
    • irreducible
  • tender mass above inguinal ligament

Thus, the strangulated bowel is intra-abdominal. This is a rare variety of strangulation.
