A double superior vena cava results from the embryological failure of regression of the left anterior cardinal vein. The common vessel between the anterior cardinal veins does not form and consequently, there is no left brachiocephalic vein.
The left anterior cardiac vein - the second superior vena cava - joins directly to the coronary sinus of the heart.
