Asthma (fitness for anaesthesia)

Authoring team

The asthmatic patient presents few problems to the anaesthetist if the condition is well-controlled.

Medication is taken at normal doses up to surgery. The only pre-operative investigation of note is the use of a peak flow meter. Possible postponement or change in medication may be warranted by:

  • a PEFR < 50% of the normal maximum
  • a PEFR less than 200l/minute
  • a significant decrease in the morning PEFR

The severe asthmatic is usually on a maximal drug regimen before assessment. A tailored dose of steroids may be used e.g. 30-60mg prednisolone once daily, before and after surgery.
