With the patient seated and the arm relaxed by the side, the lateral aspect of the shoulder joint is palpated to identify the gap between the acromium and the humeral head. The needle is then inserted from a lateral or posterolateral position and directed towards the anteromedial acromial margin. When the subacromial bursa is entered, it is possible to inject with minimal resistance.
