Management of PUO should be as follows:

  • avoid blind treatment, though "best guess" treatment may be required if the patient is septicaemic or the patient's condition is deteriorating
  • review the patient daily to find new features of history, new symptoms, new signs
  • need to search for developing focal abscesses - a surgically treatable cause
  • stop all drugs where possible

Once a specific diagnosis has been made, the treatment can be directed towards that condition.
