This reaction may be seen in patients with multibacillary forms of borderline lepromatous leprosy. Systemic symptoms are prominent, with fever, joint and kidney involvement, and severe morbidity. These effects may follow the institution of drug therapy, and are thought to be mediated by circulating immune complexes formed as a result of the fragmentation of bacilli.
