Aetiology
Lacunar strokes are most frequently due to poorly- controlled hypertension.
Perforating vessels may occlude due to lipohyalinosis, micro-atheroma or fibrin.
Lacunar strokes are rarely due to emboli from a proximal source e.g. extracranial atherosclerosis or cardiac emboli.
