There is generally a history of the foot being forced or twisted into equinus, or a crush injury.
The foot is swollen. If there is a strain then movement of the forefoot causes pain. In a fracture or dislocation all movement of the foot is resisted. There may be signs of ischaemia.
Multiple x-rays are needed in order to assess the extent of the injury.
