Tendon assessments

There are several tendons within the ankle that may be the site for pathology and require assessment:

  • Achilles tendon / tendo calcaneus:
    • assessed with the patient prone and with the feet over the side of the bed
    • inspection - for defects in the contour of the tendon
    • feel - palpate the tendon while the patient is trying to plantar-flex the ankle against resistance
    • move - bilaterally compare the force achieved by plantar-flexion in the same position

  • teno-synovitis:

Medially:

  • look and feel: along the line of the long flexor tendons behind the medial malleolus
  • plantarflex and evert the ankle joint - produces pain if the tibialis posterior tendon is inflamed

Laterally:

  • look and feel: along the line of the peroneal tendons; crepitus and swelling may be felt
  • plantarflex and invert the foot - this will produce pain along the line of the long peroneal tendons if they are inflammed
