The main aim of management is to identify primary congenital glaucoma early and treat them with surgery to normalize intraocular pressure (IOP) and minimize the damage to vision (1)Medical treatment is of little value.
Goniotomy is the treatment of choice - an incision is made into the occluded angle of the anterior chamber to restore outflow. This is successful in 70-80% of cases. The operation may fail due to the young eye mounting a fibrotic response to the artificial fistula.
Trabeculotomy is also used as a surgical option. Useful when the cornea is hazy or opaque (1)
In the absence of early treatment, blindness often develops. Early manifestation of the disease is associated with a poorer prognosis.
