This is a presentation midway between face and vertex and is diagnosed on feeling the orbital ridges on vaginal examination. It occurs approximately once in 1000 deliveries.
This presentation cannot be delivered - the diameter is too large (13.5 cm on average). The position can be corrected, either by flexing to vertex or extending to face. The other alternative is caesarian section.
