Cluster headache does not normally present difficulty in diagnosis when the symptoms have been clearly elicited. However, be certain to consider:

  • ophthalmoplegic migraine
  • a supraclinoid aneurysm
  • trigeminal neuralgia
  • other trigeminal autonomic cephalalgias (TACs) including
    • paroxysmal hemicranias
    • SUNCT (short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing) (1)
