Endocarditis due to:-
Penicillin sensitive streptococci (eg Strep viridans)  | Benzylpenicillin (or vancomycin if allergic) + low-dose gentamicin . Treat for 4/52. Stop gent after 2/52 if fully penicillin sensitive.  | 
Streptococci with reduced sensitivity to Penicillin (eg Strep. faecalis)  | Benzylpenicillin (or vancomycin if allergic) + low-dose gentamicin . Treat for at least 4/52.  | 
Staphylococcus aureus & + Staph epidermidis.  | Flocloxacillin/cloxacillin + either gentamicin or fusidic acid (or vancomycin alone if allergic). Treat for at least 4/52. Stop gent after 2/52.  |
