Documenting calls
Write down for medicolegal as well as safety reasons:
- exact times when you answered the call and when you actually saw the patient
- patient's full name, age and date of birth
- the full address and phone number
- a summary of all phone advice, signed with time and date
- a contemporaneous record of all visits - don't leave till later if at all possible
Remember or indicate the need for claim procedures (night visits, temporary residents, emergency treatment, etc).
