Congenital inguinal hernia predominantly affects boys (male:female ratio 9:1). The overall incidence is around 1-2%. The incidence rises with increasing prematurity of the child; this reflects the reduced time for obliteration of the patent processus vaginalis.

Most hernias are indirect. In boys, there is a reported tendency to occur on the right side.
