The investigation of choice is biopsy of the bone lesion, which reveals non-malignant proliferation of Langerhans cell histiocytes.

Under electron microscopy there are tennis racket-shaped Birbeck granules.

Radiology reveals a well-demarcated lytic lesion - the so called punched out lesion. A full skeletal survey may be indicated.
