There may be solitary or multiple nail involvement, of variable severity. There is usually diffuse fine pitting in transverse lines and frequent fragmentaion and ridging of nails. Nail involvement may not coincide with hair loss.

Alopecia has also been associated with twenty-nail dystrophy.
